چکیده

In this issue of Circulation, Safian et all describe their experience with directional coronary atherectomy. The procedure was successful in removing 88% of lesions, and only one of 67 patients required emergency coronary bypass surgery. However, according to life-table analysis, 30% of the lesions restenosed in the ensuing 6 months. Thus, coronary atherectomy appears to be effective and safe in selected patients, but the incidence of restenosis is high. How do the results of this study and those from other centers using this procedure compare with

Comparative angiographic quantitative analysis of the immediate efficacy of coronary atherectomy with balloon angioplasty, stenting, and rotational ablation.

Interventional cardiology has branched in two directions: devices that primarily dilate coronary stenoses and those that debulk coronary tissue. Presently the optimum coronary intervention has not been found. While patients are awaiting randomized trials, a comparison based on matched quantitative coronary analysis may be useful to evaluate results of new interventional techniques. Therefore we...

Directional Atherectomy Versus Balloon Angioplasty for Coronary Ostial and Nonostial Left Anterior Descending Coronary Artery Lesions: Results From a Randomized Multicenter Trial

Objectives. We hypothesized that atherectomy would be superior to balloon angioplasty for ostial and nonostial left anterior descending coronary artery lesions. Background. Balloon angioplasty of ostial coronary artery lesions has been associated with a lower procedural success rate and a higher rate of complications and of restenosis than angioplasty of nonostial stenoses. Directional coronary...

Influence of plaque morphology on the mechanism of luminal enlargement after directional coronary atherectomy and balloon angioplasty.

OBJECTIVE To relate the mechanism of luminal gain after directional atherectomy and balloon angioplasty to the morphological characteristics of the coronary lesions, assessed by intravascular ultrasound imaging. DESIGN Intravascular ultrasound imaging was performed before and after the revascularisation procedure to assess the contribution of wall stretching and plaque reduction in luminal ga...

Subacute narrowing of the left main coronary artery following directional atherectomy for proximal obstructive coronary artery disease.

Directional atherectomy is an alternative device for treatment of highly eccentric or proximal coronary lesions considered less suitable for balloon angioplasty. We report a patient with exuberant neointimal proliferation, extending into the left main coronary artery, following directional atherectomy of the proximal target site in the left circumflex artery. This observation indicates that pat...

Quantitative angiography after directional coronary atherectomy.

OBJECTIVE To assess by quantitative analysis the immediate angiographic results of directional coronary atherectomy. To compare the effects of successful atherectomy with those of successful balloon dilatation in a series of patients with matched lesions. DESIGN Case series. SETTING Tertiary referral centre. PATIENTS 62 patients in whom directional coronary atherectomy was attempted betwe...
